-  For expired passport

1.   Travel Document Application Form;

 2.   Current passport;

3.    Photocopy of passport's data page;

4.    Four (4) pieces passport-size photo with blue background;

 5.  Valid ID.

      -  For lost passport

1.  Travel Document Application Form;

2.  Affidavit of Lost Passport to be executed at the Consulate;

               3.  Confirmation of Lost Passport issued by the Public Security Bureau;

4.  Copy of the lost passport;

            5.  Birth Certificate and Marriage Contract (for married woman);

6.  Four (4) pieces passport-size photo with blue background, and;

7.  Valid ID.
