The Philippine Consulate General in Guangzhou accepts registration of the death of a Filipino national that occurs within its consular jurisdiction, which includes the province of Guangdong, Hunan, Hainan and Guangxi Zhuang Autonomous Region.
The release of processed documents to the applicants takes 3 working days. Applicants may choose to claim their documents in person or opt for courier service.
Please note that the transmittal of documents to the Philippine Statistics Authority (PSA) may take 3 to 6 months. The consulate will notify applicants via email with further details.
Requirements:
- Duly accomplished Report of Death form application form;
- Death certificate notarized by the Chinese Notary Public and apostilled by the Chinese Foreign Affairs Office (1 original and 3 photocopies);
- Original passport of the deceased (original and 4 copies);
- Proof of Relationship of the Next of Kin/relative (original and 4 copies);
- Valid passport of the relative/informant (original and 4 copies);
- Embalmer's Certificate (with English translation) prepared by the funeral parlor (1 original and 4 copies);
- Cremation Certificate (if cremated, with English translation) prepared by the funeral parlor (1 original and 4 copies);
- Antiseptic Certification of Entry/ Exit of Corpse (1 original and 4 copies);
- Transit/Export Permit (with English translation) issued by the Entry-Exit Inspection and Quarantine, PROC (1 original and 4 copies);
- Payment of applicable fees.
