New Passport Application

Requirements for New (Adult) Applicants

1. PSA birth certificate authenticated/apostilled by the DFA;
2. PSA marriage certificate authenticated/apostilled by the DFA (for married woman);
3. Valid Philippine issued ID (please see the list of acceptable ID’s)
4. Accomplished passport application form
5. Applicable fee

Requirements for New (Minor) Applicants

1. Personal Appearance of minor applicant and either parent or authorized adult companion;
2. Accomplished application form;
3. Report of Birth or PSA birth certificate
4. Valid passport or valid government issued IDs of either parent;
5. School ID (if applicable);
6. Applicable fee

Note: The Consulate may require additional supporting documents as may be necessary.
